REPEAT PRESCRIPTION BY E-MAIL

Lavenham (Long Melford) Surgery or Bildeston Health Centre

The current coronavirus situation means that the Lavenham pharmacy are not accepting repeat prescriptions on paper forms.  

Repeat prescriptions can be ordered by e-mail either by the patient or someone acting on their behalf. If you are unable to request a repeat prescription by this method contact the parish council and they will find someone who will be able to e-mail it for you.

The e-mail address for repeat prescriptions is:

Your e-mail needs to include the following information:

Your full name.
Your NHS number. **
Date of birth.
Your full postal address and post code.
A list of just the items you need for the next month. **
A request for the repeat prescription to be sent to the Lavenham Pharmacy.

**  These details can be found either on the slip that you received with your last prescription or from the pharmacy label on the packaging.

Allow two days before making arrangements to collect your prescription. 

If you are unable to collect or make arrangements for collection, then either ring the pharmacy 01787 247284 who may be able to deliver, or the Lavenham Good Neighbours 07922 850082 who will arrange for someone to collect it on your behalf.
